- Get prepared to register. You will need an electronic copy of a valid US government issued photo such as a license or passport. Proof of a Maryland address. An scanned copy of a recent photo and the last 4 digits of your social security. Please note that if you are a hospice patient, you may have your attending submit a letter on hospice letterhead. You will then need to go to this page to register: MMCC Registration Link
- After completing the registration you will receive a confirmation by email.
- You will then need to receive a written certification from a registered provider. Currently we are registered as a provider and can complete this certification. You must first be evaluated for this to happen. After your evaluation, if you meet the criteria a certification will be completed.
- Once this is successfully completed, you must visit a Maryland licensed dispensary. After verifying your registration you should be able to receive a 30 day supply of medical marijuana. If your certification is not used within 120 days of issuance it will become null and void.
